Registration for each player and team is mandatory!
TEAMS WHO HAVE NOT SUBMITTED THEIR OFFICIAL LINEUP/WAIVER FORMS WILL NOT BE SCHEDULED TO PLAY!
Only teams that have registered and submitted their waiver forms will be scheduled moving forward. Deadline of submission will be this Saturday November 5, 2016 @ midnight.

Fill out the Team Registration Form and email it to info@pbam.ca (or any PBAM Officer listed on the website) immediately to complete your team registration. Make sure that everyone listed on your roster has registered in person and has PBAM Membership card or they will not be able to play!

Terms and Conditions
Above indicated fees are calculated based on cost of operation per division.
Fees indicated are based on the number of teams registered, number of games, referees (single or double), regular season and play-offs, etc.
A payment schedule will be indicated on each individual team’s invoice.
A team’s failure to remit payment on or before the scheduled dates will NOT be allowed to play.
Defaulting on the team registration fee in part or in whole will be subject to PBAM sanctions;
➢ Association or Team will not be permitted to field or register a team in same division(s) in upcoming season(s).
➢ Player(s) that defaulted on their team’s registration fee will be prevented from participating on any PBAM related activities. The team’s management will be required to furnish the name(s) of the individuals to be sanctioned.

To players who have not yet registered , it will be done on Game Day .
Come early to avoid the line-up and get your name in to be able to play.
RULE:
To register, bring the following:
Present minimum 2 pieces of ID:
• (Identification with picture) and another with your Date of Birth
• Proof of Filipino bloodline may be required for those of mixed races.
– Picture will be taken as part of the registration.
– Players, coaches, teams NOT registered in the PBAM database.. NO GAME participation.
